Bring your expertise to a rewarding role providing compassionate care to patients in their own homes. As a Home Health Physical Therapist, youll play a vital part in helping individuals regain their independence and improve their quality of life.

Schedule & Setting:

• Full-time, 13-week contract
• Monday Friday, 8:00 AM 5:00 PM
• Approximately 2532 points per week (57 patient visits daily)
• Field-based, serving patients in their homes
• Daily travel within assigned territory (mileage reimbursement provided)

Qualifications:

• Active Physical Therapist license in good standing for the state
• At least 1 year of recent experience in Home Health Physical Therapy (field experience preferred)
• Proficiency with Home Care Home Base (HCHB) electronic documentation preferred
• Experience with OASIS assessments required
• Reliable transportation, valid drivers license, and auto insurance
• Strong clinical skills, independent time management, and organizational abilities

Key Responsibilities:

• Perform comprehensive physical therapy assessments, including OASIS when required
• Develop, implement, and adjust personalized therapy plans focused on functional mobility, balance, gait, and pain management
• Deliver skilled interventions: therapeutic exercise, neuromuscular re-education, transfer and gait training, and fall prevention
• Monitor patient progress, collaborate with care team members, and update care plans as needed
• Educate patients and caregivers on exercises, assistive device use, and safety measures
• Accurately complete documentation in Home Care Home Base (HCHB)
• Attend interdisciplinary meetings and comply with all policies and regulatory guidelines

$2,016 - $2,132/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
