An opportunity is available for a skilled Home Health Physical Therapist to provide in-home physical therapy services, focusing on enhancing patients mobility, strength, balance, and overall independence. This role involves working independently to conduct evaluations, develop personalized care plans, and collaborate with healthcare team members to promote positive health outcomes. Strong organizational skills and a patient-centered approach are essential for success in this field-based position.

This is a full-time contract position lasting approximately 13 weeks, offering a Monday through Friday schedule from 8:00 AM to 5:00 PM. Expect to manage a caseload of about 2532 points weekly, typically seeing 57 patients each day within a designated territory. Reliable transportation is required as daily travel to patient homes is part of the job, with mileage reimbursement provided.

Key Responsibilities:

• Perform comprehensive physical therapy evaluations, including OASIS assessments as required
• Design and implement individualized treatment plans targeting mobility, strength, balance, and safety
• Deliver skilled interventions such as therapeutic exercises, neuromuscular re-education, gait and transfer training, and fall prevention strategies
• Monitor patient progress, adjust care plans as needed, and communicate updates to physicians and care teams
• Educate patients and caregivers on exercise routines, safety measures, assistive device use, and home exercise programs
• Maintain accurate and timely documentation using Home Care Home Base (HCHB)
• Collaborate with nurses, occupational therapists, speech therapists, social workers, and home health aides to coordinate care
• Comply with all regulatory guidelines and agency policies

Qualifications:

• Active Physical Therapist license in good standing within the state of practice
• Minimum of one year of recent home health physical therapy experience, with field experience preferred
• Proficiency with Home Care Home Base (HCHB) and OASIS documentation required
• Strong clinical judgment and functional assessment capabilities
• Valid drivers license, reliable transportation, and automobile insurance
• Excellent organizational, communication, and time management skills
• Ability to work autonomously and manage a flexible schedule
