A rewarding opportunity is available for a skilled Home Health Physical Therapist (PT) to provide compassionate, patient-centered care in a community setting. This role involves delivering therapeutic physical therapy services directly in patients homes, promoting improved mobility, strength, balance, and overall functional independence.

Key responsibilities include conducting thorough evaluations, performing OASIS assessments, developing personalized treatment plans, and delivering skilled interventions like therapeutic exercise, gait training, and fall prevention. The therapist communicates progress to physicians and interdisciplinary team members, educates patients and caregivers, and ensures accurate and timely documentation using Home Care Home Base (HCHB). Collaboration with nurses, occupational therapists, speech-language pathologists, social workers, and home health aides is essential to coordinate comprehensive care.

Qualifications:

• Active Physical Therapist license in the state (in good standing)
• Minimum one year of recent Home Health PT experience; fieldwork preferred
• Proficiency or experience with Home Care Home Base (HCHB) strongly preferred
• OASIS assessment experience required
• Strong clinical decision-making and functional assessment skills
• Reliable transportation, valid drivers license, and auto insurance
• Ability to work independently with excellent organizational and communication skills

This full-time, 13-week contract position follows a Monday to Friday schedule from 8:00 AM to 5:00 PM, with a caseload averaging 2532 points weekly (about 57 patient visits per day). Daily travel to patient homes within the assigned territory is required, with mileage reimbursement provided.
