Schedule:

• Monday Friday, 8:00 AM 5:00 PM
• Full-time, 13-week contract

Key Responsibilities:

• Perform comprehensive physical therapy evaluations, including OASIS assessments for Start of Care, Resumption, Recertification, and Discharge when applicable.
• Develop individualized care plans tailored to improve functional mobility, strength, balance, and safety.
• Administer evidence-based interventions, such as therapeutic exercises, neuromuscular re-education, gait and transfer training, and fall prevention strategies.
• Assess and document patient progress, updating plans of care as needed and collaborating with physicians and the interdisciplinary team.
• Educate patients and caregivers on home exercise programs, safe mobility, the use of assistive devices, and injury prevention.
• Utilize Home Care Home Base (HCHB) for accurate, timely electronic documentation.
• Participate in interdisciplinary meetings, case conferences, and ongoing education.

Desired Qualifications:

• Current Physical Therapist (PT) license in good standing in the practicing state
• At least one year of recent home health physical therapy experience (field experience strongly preferred)
• Proficiency with Home Care Home Base (HCHB) and mandatory OASIS assessment experience
• Reliable vehicle, valid drivers license, and auto insurance for daily patient travel
• Excellent clinical judgment, organization, and time management abilities

$2,299 - $2,432/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
