Job Description
Deliver compassionate, skilled care to patients in the comfort of their homes as a Home Health Physical Therapist. This position supports individuals in improving mobility, strength, balance, and functional independence through evidence-based physical therapy interventions and collaborative care. Youll thrive if you are highly self-motivated, organized, and comfortable working independently, while ensuring high standards of patient-centered care.
Schedule:
- Monday to Friday, 8:00 AM 5:00 PM
- 13-week contract, full-time commitment
Key Responsibilities:
- Complete thorough physical therapy evaluations, including OASIS assessments as required.
- Develop and implement individualized treatment plans emphasizing mobility, balance, gait training, pain management, safety, and functional goals.
- Deliver hands-on interventions, such as therapeutic exercise, neuromuscular re-education, transfer and gait training, and fall prevention strategies.
- Assess patient progress, update plans of care, and communicate patient status updates promptly to physicians and the multidisciplinary team.
- Provide education to patients and caregivers regarding home exercise programs, safety strategies, and use of assistive devices.
- Document all visits and interventions accurately and efficiently using Home Care Home Base (HCHB).
- Collaborate extensively with nurses, occupational therapists, speech-language pathologists, and other healthcare professionals to ensure integrated patient care.
- Participate in case conferences and ongoing professional development as needed.
- Maintain compliance with agency policies, regulatory standards, and Medicare home health guidelines.
Qualifications:
- Current, active Physical Therapist (PT) license in good standing for the state of practice
- Minimum of 1 year of recent Home Health Physical Therapy experience required
- Working experience with HCHB preferred; OASIS expertise required
- Demonstrated clinical judgment and strong functional assessment abilities
- Reliable transportation, valid drivers license, and active auto insurance
- Excellent communication, organization, and independent time management skills

Compensation for this position ranges from $28.06 to $60.00. Eligible candidates may also receive healthcare benefits, housing, and meals/incidentals. Full details of our healthcare benefits plan can be found here. This posting is open for 60 days after the posting date.
