Join a dynamic healthcare team as a Home Health Physical Therapist (PT), delivering essential physical therapy services directly in patients' homes. This full-time, 13-week contract role offers the opportunity to make a meaningful impact on patient mobility, strength, balance, and independence while working independently within a supportive framework.

Key Responsibilities:

• Perform comprehensive physical therapy evaluations, including OASIS assessments as required.
• Develop and implement individualized plans of care targeting mobility, strength, gait, pain management, and safety.
• Deliver skilled interventions such as therapeutic exercise, neuromuscular re-education, gait and transfer training, and fall prevention strategies.
• Monitor patient progress, adjust treatment plans accordingly, and communicate updates with physicians and care teams.
• Educate patients and caregivers on exercises, safety measures, assistive device use, and home exercise programs.
• Maintain accurate and timely documentation in Home Care Home Base (HCHB).
• Collaborate with interdisciplinary team members to ensure coordinated, patient-centered care.
• Participate in case conferences and adhere to all agency policies and Medicare home health guidelines.

Qualifications:

• Active Physical Therapist license in good standing for the state of practice.
• Minimum of one year recent home health physical therapy experience; field experience preferred.
• Proficiency with OASIS assessments required; familiarity with Home Care Home Base (HCHB) preferred.
• Strong clinical decision-making, functional assessment, and communication skills.
• Reliable transportation, valid drivers license, and auto insurance to manage daily travel within assigned territory.
• Ability to work independently with excellent time management and organization.

Schedule & Location:

• Monday through Friday, 8:00 AM to 5:00 PM
• Field-based role requiring daily travel to patient homes within Hyattsville, MD area.
• Mileage reimbursement provided.
