An experienced Home Health Physical Therapist (PT) is needed to deliver skilled physical therapy services directly to patients in their homes. This full-time, 13-week contract role involves providing patient-centered care aimed at improving functional mobility, strength, balance, and overall independence. The position requires autonomous work in a field-based setting, conducting comprehensive evaluations and developing individualized treatment plans.

Key Responsibilities:

• Perform thorough physical therapy evaluations, including required OASIS assessments.
• Design and implement tailored PT plans focusing on therapeutic exercise, neuromuscular re-education, gait and transfer training, pain management, and fall prevention.
• Monitor patient progress, adjust treatments as necessary, and communicate updates to physicians and care teams.
• Educate patients and caregivers on exercises, safety protocols, assistive device use, and home exercise programs.
• Maintain accurate and timely documentation in Home Care Home Base (HCHB).
• Collaborate with interdisciplinary healthcare professionals to ensure coordinated care.
• Participate in case conferences, team meetings, and required continuing education.
• Comply with agency policies, regulatory standards, and Medicare home health guidelines.

Qualifications:

• Active Physical Therapist license in good standing within the state of practice.
• Minimum of one year recent home health physical therapy experience, with field experience preferred.
• Proficiency with Home Care Home Base (HCHB) preferred; OASIS experience required.
• Strong clinical decision-making and functional assessment capabilities.
• Reliable transportation with valid drivers license and auto insurance.
• Ability to work independently and manage a flexible schedule.
• Excellent communication, time management, and organizational skills.

Location Overview: This position is based in Hughes, Arkansas, a welcoming community known for its small-town charm and access to local amenities. The role involves daily travel within an assigned territory, providing a diverse and dynamic work environment to impact patients lives directly in their homes.

Additional Details:

• Schedule: Monday through Friday, 8:00 AM 5:00 PM.
• Caseload: Approximately 2532 points weekly, typically 57 patient visits per day.
• Mileage reimbursement provided.
