Deliver expert physical therapy services as a Home Health Physical Therapist (PT) providing patient-centered care in home environments. This role involves conducting thorough evaluations, creating tailored treatment plans, and collaborating with an interdisciplinary healthcare team to enhance patients mobility, strength, balance, and independence. Schedule/Hours:
- Monday Friday
- 8:00 AM 5:00 PM
Key Responsibilities:
- Perform comprehensive physical therapy evaluations including OASIS assessments as required.
- Develop and implement individualized treatment plans focusing on functional mobility, strength, gait, pain management, and safety.
- Provide skilled interventions such as therapeutic exercise, neuromuscular re-education, gait and transfer training, and fall prevention.
- Monitor patient progress, adjust treatments accordingly, and communicate updates to physicians and care teams.
- Educate patients and caregivers on exercises, safety strategies, assistive device use, and home exercise programs.
- Maintain accurate, timely electronic documentation using Home Care Home Base (HCHB).
- Collaborate with healthcare professionals including nurses, occupational therapists, social workers, and home health aides.
- Participate in case conferences, interdisciplinary meetings, and continuing education.
- Comply with agency policies, Medicare guidelines, and regulatory standards.
Qualifications / Desired Experience:
- Active Physical Therapist license in good standing in the practicing state
- Minimum 1 year of recent home health physical therapy experience; field experience preferred
- Proficiency with Home Care Home Base (HCHB) software preferred
- Experience with OASIS assessments required
- Strong clinical reasoning and functional assessment skills
- Reliable transportation, valid drivers license, and auto insurance
- Ability to work independently and manage a flexible schedule
- Excellent communication, organizational, and time-management skills
