Step into a meaningful role as a Home Health Physical Therapist, where youll deliver compassionate, high-impact care to patients in their homes. This full-time, 13-week contract position is ideal for a skilled clinician eager to foster functional independence and quality of life in a field-based environment.

Schedule and Structure:

• MondayFriday, 8:00 AM5:00 PM
• Approximately 2532 points per week (typically 57 patient visits daily, based on visit types)
• Travel required within your assigned territory (mileage reimbursement provided)

Position Highlights:

• Conduct comprehensive patient evaluations, including OASIS assessments for Start of Care, Resumption, Recertification, and Discharge as needed.
• Create and implement tailored plans of care addressing mobility, strength, balance, pain management, and patient safety.
• Deliver skilled interventions such as therapeutic exercise, gait and transfer training, neuromuscular re-education, and fall prevention.
• Closely monitor and document patient progress within the Home Care Home Base (HCHB) electronic system.
• Collaborate with physicians, nurses, and other healthcare professionals to ensure continuity and quality of care.
• Educate patients and caregivers on safe practices, exercise regimens, and use of assistive equipment.
• Maintain compliance with agency policies, regulatory standards, and Medicare home health guidelines.

Desired Experience & Qualities:

• Active Physical Therapist (PT) license in good standing
• At least 1 year of recent home health PT experience (field-based work preferred)
• Working knowledge of HCHB and required proficiency in OASIS
• Strong assessment, clinical reasoning, organizational, and communication skills
• Reliable transportation, valid drivers license, and up-to-date auto insurance
