Job Description
Deliver skilled, patient-centered care as a Home Health Physical Therapist, supporting individuals as they regain strength, mobility, and independence in their own homes. This position offers a 13-week full-time contract, providing field-based care within an assigned territory, Monday through Friday from 8:00 AM to 5:00 PM.
Key Responsibilities:
- Conduct thorough physical therapy assessments, utilizing OASIS as appropriate.
- Develop and implement tailored treatment plans addressing mobility, strength, pain, balance, and safety.
- Deliver interventions including therapeutic exercise, gait and transfer training, neuromuscular re-education, and fall prevention strategies.
- Educate and empower patients and caregivers on exercise programs, safety, and use of assistive devices.
- Closely monitor progress and make clinical modifications as needed, communicating effectively with healthcare team members.
- Maintain accurate, timely documentation in Home Care Home Base (HCHB).
- Collaborate with nurses, occupational therapists, SLPs, social workers, and aides for integrated patient care.
- Participate in case conferences, interdisciplinary meetings, and ongoing training.
- Uphold all regulatory compliance, agency policies, and Medicare guidelines.

- Monday Friday, 8:00 AM 5:00 PM
Your Qualifications:
- Active, unencumbered Physical Therapist (PT) license in the state of practice
- Minimum 1 year of recent Home Health Physical Therapy experience
- OASIS experience required; HCHB proficiency preferred
- Strong assessment, organization, and communication skills
- Reliable transportation, valid drivers license, and auto insurance
- Self-motivated with the ability to work independently
