Job Description
Experienced and compassionate physical therapists are needed to provide outstanding home health care to patients aiming to improve mobility, strength, and independence. This full-time, 13-week contract involves working Monday through Friday, 8:00 AM 5:00 PM, with a typical caseload of approximately 2532 points per weekgenerally translating to 57 patient visits per day in a field-based, home setting.
Key Responsibilities:
- Conduct thorough physical therapy evaluations, including OASIS assessments for applicable visits.
- Design and execute individualized care plans focused on mobility, strength, balance, and functional independence.
- Deliver skilled interventions such as therapeutic exercises, gait and transfer training, neuromuscular re-education, and fall prevention.
- Regularly evaluate patient progress, adjusting therapy plans and collaborating with physicians and interdisciplinary team members.
- Educate patients and caregivers on safety strategies, exercise protocols, use of assistive devices, and ongoing home exercise programs.
- Maintain complete, timely electronic documentation using Home Care Home Base (HCHB).
- Participate in case conferences and interdisciplinary meetings as required, ensuring up-to-date, coordinated patient care.
- Strictly adhere to agency policies, Medicare, and home health guidelines.
Qualifications:
- Active, unrestricted Physical Therapist (PT) license in North Carolina.
- At least one year of recent home health physical therapy experience (field work preferred).
- Required OASIS experience; familiarity with Home Care Home Base (HCHB) strongly preferred.
- Demonstrated clinical assessment and decision-making skills.
- Reliable personal transportation, valid drivers license, and auto insurance.
- Strong communication, organization, and time management abilities; self-starter who thrives with independent schedules.
