Job Description

A Home Health Physical Therapist opportunity is available in Hartford, CT, offering a dynamic role providing in-home care to patients on a 13-week contract. This position involves delivering specialized physical therapy services directly in patients residences, supporting their journey toward improved mobility and quality of life.

Key Responsibilities:

  • Perform thorough initial patient assessments to determine physical conditions and functional capabilities.
  • Design tailored treatment plans aligned with patient needs and physician directives.
  • Implement therapeutic exercises, manual therapy, and modalities to enhance strength, range of motion, and independence.
  • Educate patients and caregivers on proper exercise techniques, transfers, and assistive device use.
  • Track and document patient progress, adjusting interventions as appropriate.
  • Collaborate closely with physicians, nurses, and interdisciplinary team members to ensure cohesive care.
  • Maintain accurate electronic medical records in compliance with regulatory standards.
  • Uphold ethical, professional, and legal standards of practice in all patient interactions.

Qualifications and Desired Experience:

  • Bachelors or Masters degree in Physical Therapy from an accredited program.
  • Current physical therapist license or eligibility for licensure in the state.
  • Experience delivering physical therapy in a home health setting.
  • Proficiency with HomeCare HomeBase (HCHB) electronic medical records system preferred.
  • Strong clinical reasoning and communication skills.
  • Compassionate, patient-focused approach with ability to motivate and engage patients.
  • Knowledge of Medicare regulations and home health documentation.
  • Valid drivers license and reliable transportation for travel to patient homes.
