Experienced and compassionate physical therapists are sought to deliver skilled care directly to patients in their homes, promoting improved mobility, safety, and independence. This full-time, 13-week contract role offers the opportunity to make a meaningful impact as part of an interdisciplinary home health team.

Schedule & Structure:

• Monday Friday, 8:00 AM 5:00 PM
• 13-week contract, full-time
• Caseload: Approximately 2532 points weekly (about 57 patient visits daily)
• Field-based setting (visiting patient homes)
• Electronic documentation: Home Care Home Base (HCHB)
• Daily local travel (mileage reimbursement provided)

Desired Skills & Qualifications:

• Active PT license in good standing for the state
• At least 1 year of recent home health physical therapy experience
• Strong familiarity with OASIS documentation (required)
• Experience using Home Care Home Base (HCHB) preferred
• Excellent clinical assessment and decision-making abilities
• Reliable transportation, valid drivers license, and current auto insurance
• Self-motivated, with solid time management and organizational skills
• Strong written and verbal communication; collaborative approach to care

Key Responsibilities:

• Conduct thorough PT evaluations, including OASIS for appropriate assessments
• Develop and adjust individualized therapy plans aimed at restoring function, strength, and safe mobility
• Implement therapeutic exercises, gait and transfer training, and fall prevention strategies
• Provide patient and caregiver education on safety, assistive devices, and home exercise programs
• Keep accurate, timely records in HCHB
• Collaborate with nurses, therapists, social workers, and home health aides for comprehensive care
• Participate in case conferences and ongoing professional development
• Ensure compliance with Medicare guidelines and agency policies

$2,129 - $2,252/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
