A dedicated Physical Therapist with home health experience is needed to provide expert care to patients within their residences on a 13-week contract basis. This role focuses on delivering personalized therapeutic interventions that enhance patients mobility, strength, and independence in a home health setting.

Key Responsibilities:

• Perform comprehensive initial patient assessments evaluating physical conditions and mobility.
• Design and implement individualized treatment plans in alignment with patient goals and physician directives.
• Administer therapeutic exercises, manual therapy, and modalities to promote recovery.
• Educate patients and families on exercises, transfers, and use of assistive devices.
• Monitor progress and update treatment plans accordingly.
• Communicate effectively with physicians, nurses, and the healthcare team to coordinate care.
• Document patient care accurately using electronic medical records, ensuring compliance with regulatory standards.
• Adhere strictly to ethical and professional standards, as well as state and federal regulations.

Qualifications:

• Bachelors or Masters degree in Physical Therapy from an accredited program.
• Current state license or eligibility for Physical Therapist licensure.
• Experience working in a home health environment.
• Proficient with HomeCare HomeBase (HCHB) electronic medical records.
• Strong clinical judgment and critical thinking skills.
• Excellent communication and interpersonal skills.
• Compassionate, patient-focused approach with motivational abilities.
• Knowledge of Medicare regulations pertinent to home health.
• Valid drivers license and reliable transportation for travel to patient homes.
