Deliver patient-centered care as an experienced Home Health Physical Therapist, offering vital services in a home-based setting for individuals seeking to regain independence, mobility, and strength. This role focuses on providing comprehensive physical therapy to adults within their residences, supporting recovery and functional improvement.

Schedule:

• Full-time, 13-week contract position
• Monday through Friday, 8:00 AM 5:00 PM
• Daily travel to patient homes in your assigned area with mileage reimbursement

Key Responsibilities:

• Complete thorough physical therapy evaluations, performing OASIS assessments for Start of Care, Recertification, Resumptions, and Discharge as necessary
• Design and deliver individualized therapy plans that target mobility, strength, balance, gait training, and pain management
• Implement therapeutic interventions, including neuromuscular re-education and fall prevention strategies
• Continuously monitor, document, and adjust treatment plans based on patient progress using the Home Care Home Base (HCHB) electronic documentation system
• Educate patients and caregivers on exercise regimens, use of assistive devices, and safety in the home environment
• Collaborate with a multidisciplinary healthcare team, participating in conferences and coordinating holistic care for optimal patient outcomes
• Maintain compliance with agency policies, Medicare guidelines, and regulatory standards

Qualifications:

• Active, unencumbered Physical Therapist (PT) license for the state of practice
• At least one year of recent field-based Home Health PT experience is required
• Strong working knowledge of OASIS assessments and HCHB preferred
• Excellent organizational, self-management, and communication skills
• Valid drivers license, reliable transportation, and auto insurance
