Job Description

Deliver high-quality patient-centered care as a vital member of a dynamic home health team, providing skilled physical therapy services in patients homes. This contract role offers an opportunity to work independently while helping individuals maximize mobility, function, and safety as they recover or manage health conditions. The position is a full-time, 13-week contract, Monday through Friday, 8:00 AM 5:00 PM, with a caseload of approximately 2532 points per week (typically five to seven patient visits per day depending on visit type).

Key Qualifications:

  • Current and active Physical Therapist (PT) license in good standing
  • Minimum of one year of recent Home Health Physical Therapy experience (field work strongly preferred)
  • Familiarity with Home Care Home Base (HCHB) is preferred
  • Required experience with OASIS documentation and assessments
  • Strong clinical, functional assessment, and decision-making skills
  • Reliable transportation, valid drivers license, and proof of auto insurance
  • Outstanding organizational, time management, and communication abilities
  • Ability to work independently and flexibly in a field-based environment

Primary Responsibilities:

  • Perform comprehensive physical therapy evaluations, including OASIS assessments when indicated
  • Develop and implement tailored treatment plans focused on improving function, mobility, and safety
  • Provide skilled therapies such as exercise, gait and transfer training, neuromuscular re-education, and fall prevention
  • Educate patients and caregivers in exercises, safety strategies, and the proper use of assistive devices
  • Accurately complete timely electronic documentation using HCHB
  • Communicate and coordinate with interdisciplinary care team members to ensure the best outcomes
  • Participate in case conferences and maintain compliance with agency, Medicare, and regulatory standards
