Make a meaningful difference by providing essential rehabilitation services as a Home Health Physical Therapist. This contract opportunity centers on delivering skilled physical therapy care to patients in the comfort of their homes, supporting clients to improve their mobility, strength, balance, and independence. Leverage your expertise to carry out patient-focused evaluations and interventions that foster optimal outcomes, while working autonomously and in close collaboration with interdisciplinary team members.

Schedule:

• Full-time, 13-week contract
• Monday Friday, 8:00 AM 5:00 PM
• Approximately 2532 points per week (typically 57 patient visits per day)
• Daily travel within an assigned territory (mileage reimbursement provided)

Key Responsibilities:

• Complete thorough home health physical therapy evaluations, including OASIS assessments as required
• Create and implement individualized therapy plans that target mobility, gait, pain, balance, and safety
• Provide skilled therapeutic interventions: exercise, gait training, fall prevention, and more
• Track patient progress, update treatment plans, and communicate with the healthcare team
• Educate patients and caregivers about exercises, assistive devices, and maintaining safety at home
• Utilize Home Care Home Base (HCHB) for timely, accurate electronic documentation
• Collaborate with nurses, occupational therapists, speech therapists, and others to ensure holistic care
• Attend case conferences, meetings, and participate in ongoing professional education
• Remain compliant with agency protocols, Medicare guidelines, and state regulations

Desired Qualifications:

• Active Physical Therapist (PT) license in good standing
• At least 1 year of recent home health PT experience (field-based strongly preferred)
• Strong working knowledge of HCHB and required use of OASIS
• Excellent communication, organizational, and clinical assessment abilities
• Reliable transportation, valid drivers license, and current auto insurance
