Deliver skilled therapy in a meaningful setting, working as a Home Health Physical Therapist. This opportunity places you alongside patients and families, providing direct care in their homes to promote mobility, balance, strength, and overall independence.

Schedule:

• Monday Friday, 8:00 AM 5:00 PM
• Full-time, 13-week contract

Key Responsibilities:

• Conduct comprehensive physical therapy evaluations, including OASIS assessments (Start of Care, Resumption, Recertification, and Discharge as needed).
• Develop and implement individualized care plans focused on functional improvement and safety.
• Administer skilled interventions (therapeutic exercise, neuromuscular re-education, gait/transfer training, and fall prevention).
• Monitor patient progress, updating care plans and collaborating with other healthcare professionals.
• Educate patients and caregivers on safe mobility, assistive device use, and tailored home exercise programs.
• Document patient encounters thoroughly and accurately using Home Care Home Base (HCHB).
• Engage in interdisciplinary team meetings, case conferences, and ongoing professional development.
• Ensure adherence to agency policies, regulatory standards, and Medicare home health guidelines.
• Travel daily to patient homes within your assigned area (mileage reimbursement provided).

Minimum Qualifications:

• Active Physical Therapist (PT) license in good standing.
• At least 1 year of recent home health PT experience (field-based preferred).
• Familiarity with Home Care Home Base (HCHB) electronic documentation.
• Strong experience with OASIS assessments.
• Proven clinical decision-making skills and proficiency in conducting functional assessments.
• Reliable transportation, valid drivers license, and auto insurance.
• Strong communication, time management, and organization skills; comfortable working independently.

$2,414 - $2,554/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
