An opportunity is available for a skilled Home Health Physical Therapist (PT) to deliver expert care directly in patients homes within Greenville, NC. This full-time, 13-week contract role offers the chance to apply your expertise in physical therapy by enhancing patient mobility, strength, balance, and daily function in a supportive, autonomous field-based setting.
Key Responsibilities:
- Conduct comprehensive physical therapy evaluations and OASIS assessments as required.
- Develop and implement patient-centered treatment plans focusing on functional mobility, gait, pain management, strength, balance, and fall prevention.
- Deliver skilled interventions including therapeutic exercise, neuromuscular re-education, gait and transfer training.
- Monitor patient progress, adjust treatment plans as needed, and maintain clear communication with physicians and the care team.
- Educate patients and caregivers on exercises, safety, assistive device use, and home exercise programs.
- Accurately document all care activities using Home Care Home Base (HCHB).
- Collaborate with interdisciplinary healthcare professionals to ensure coordinated, high-quality care.
- Comply with all agency policies, Medicare home health guidelines, and regulatory standards.
Qualifications and Desired Experience:
- Active Physical Therapist license in good standing for the state of practice.
- Minimum 1 year of recent home health physical therapy experience; field experience preferred.
- Experience with OASIS required; familiarity with Home Care Home Base (HCHB) preferred.
- Strong clinical judgment, functional assessment capabilities, and independent work ethic.
- Valid drivers license, reliable transportation, and auto insurance necessary.
- Excellent communication, organizational, and time management skills.

Schedule:
- Monday through Friday
- 8:00 AM to 5:00 PM
- Caseload of approximately 2532 points weekly (57 patient visits daily)
