This opportunity seeks a dedicated Home Health Physical Therapist (PT) to deliver high-quality, patient-centered therapy services focused on improving mobility, strength, balance, and overall functional independence.

Key Responsibilities:

• Conduct comprehensive physical therapy evaluations including OASIS assessments for Start of Care, Resumption, Recertification, and Discharge when applicable
• Develop and implement individualized treatment plans targeting functional mobility, strength, pain management, gait, and fall prevention
• Provide skilled interventions such as therapeutic exercises, neuromuscular re-education, gait and transfer training
• Monitor patient progress and communicate care plan updates to physicians and interdisciplinary team members
• Educate patients and caregivers on home exercise programs, safety, use of assistive devices
• Maintain timely, accurate documentation using Home Care Home Base (HCHB) system
• Collaborate across disciplines including nursing, occupational therapy, speech therapy, and social work
• Adhere to home health agency policies, Medicare guidelines, and regulatory standards

Qualifications:

• Active Physical Therapist license in the state (in good standing)
• Minimum one year recent home health PT experience (field experience preferred)
• Experience with OASIS assessments required; proficiency in HCHB preferred
• Strong clinical judgment and functional assessment skills
• Reliable transportation with valid drivers license and insurance
• Exceptional communication, time management, and ability to work independently

Schedule:

• Full-time contract, 13 weeks
• Monday to Friday, 8:00 AM 5:00 PM
• Caseload of approximately 2532 points weekly (about 57 patient visits/day)
