Job Description

An opportunity is available for an experienced and compassionate Home Health Physical Therapist to make a direct impact through one-on-one patient care in a home setting. This contract position is ideal for a motivated clinician ready to deliver skilled therapy services and foster independence for patients recovering or managing chronic conditions.

Schedule:

  • Full-time, 13-week contract
  • Monday Friday, 8:00 AM 5:00 PM
  • Caseload: Approximately 2532 points per week (typically 57 patient visits daily)
  • Field-based role (patient homes); daily travel required within the assigned territory

Key Responsibilities:

  • Complete thorough physical therapy evaluations, including OASIS assessments, as necessary
  • Develop and execute individualized care plans focused on mobility, balance, strength, gait training, and pain reduction
  • Provide therapeutic interventions including exercise, neuromuscular re-education, fall prevention, and training in the use of assistive devices
  • Continuously assess patient progress, adjusting care plans and collaborating with providers and the interdisciplinary team
  • Educate patients and caregivers on safety practices, home exercise sessions, and proper equipment use
  • Document care using Home Care Home Base (HCHB) system accurately and in a timely manner
  • Attend case conferences, interdisciplinary meetings, and mandated trainings
  • Adhere to professional, regulatory, and Medicare home health standards

Qualifications:

  • Current, active Physical Therapist licensure in the state of practice
  • At least 1 year of recent home health PT experience (fieldwork strongly preferred)
  • Experience with OASIS assessment and electronic documentation systems (HCHB proficiency preferred)
  • Reliable transportation with a valid drivers license and proof of auto insurance
  • Excellent independent decision-making, organizational, and communication skills
