Job Description

Provide compassionate, high-quality physical therapy care in patients homes as a Home Health Physical Therapist on a 13-week contract in Graniteville, SC. This role offers the opportunity to make a meaningful impact by evaluating, treating, and empowering patients recovering or managing conditions in a home health setting.

Responsibilities:

  • Perform comprehensive initial assessments of patients physical conditions, mobility, and functional abilities.
  • Develop individualized treatment plans aligned with patient goals and physician orders.
  • Administer therapeutic exercises, manual therapy, and modalities to improve strength, range of motion, and independence.
  • Educate patients and families on exercise techniques, safe transfers, and assistive device use.
  • Monitor patient progress and adjust therapy plans as needed.
  • Communicate effectively with physicians, nurses, and interdisciplinary team members to coordinate care.
  • Maintain accurate, timely electronic medical records and ensure compliance with regulations.
  • Adhere to ethical standards and state/federal guidelines.

Qualifications and Desired Experience:

  • Bachelors or Masters degree in Physical Therapy from an accredited institution.
  • Current state license or eligibility to practice as a Physical Therapist.
  • Experience working in a home health environment.
  • Proficiency with electronic medical record software, preferably HomeCare HomeBase (HCHB).
  • Strong clinical assessment and critical thinking abilities.
  • Excellent communication and interpersonal skills.
  • Understanding of Medicare documentation and regulations.
  • Valid drivers license and reliable transportation.
