This opportunity is ideal for a skilled and compassionate Physical Therapist dedicated to helping individuals regain mobility, strength, balance, and functional independence, all while working one-on-one and independently within a field-based home health setting.

Schedule:

• Monday Friday, 8:00 AM 5:00 PM
• 13-week contract
• Approximately 2532 points per week (typically 57 patient visits per day, visit type dependent)
• Daily travel within an assigned territory (mileage reimbursement provided)

Core Responsibilities:

• Complete comprehensive physical therapy evaluations, including all required OASIS assessments (Start of Care, Resumption, Recertification, Discharge).
• Develop and implement individualized plans of care focused on functional improvement, pain management, and patient safety.
• Provide hands-on interventions such as therapeutic exercises, neuromuscular re-education, gait training, and fall prevention strategies.
• Evaluate and monitor patient progress, ensuring timely adjustments to treatment plans and communication with physicians and interdisciplinary team members.
• Educate patients and caregivers on home programs, proper use of assistive devices, and safety techniques.
• Maintain accurate documentation in Home Care Home Base (HCHB) and participate in case conferences and interdisciplinary meetings.
• Uphold regulatory compliance and follow all Medicare home health guidelines.

Required Qualifications:

• Active and valid Physical Therapist (PT) license in the state of practice.
• At least 1 year of recent home health PT experience; field experience strongly preferred.
• OASIS assessment experience is required; HCHB documentation proficiency preferred.
• Reliable transportation, drivers license, and auto insurance.
