Make a meaningful impact by delivering high-quality, patient-centered care as a Home Health Physical Therapist. This contract opportunity calls for a skilled clinician who thrives working independently and is dedicated to helping patients regain their mobility and independence in the comfort of their homes.
Schedule:
- Full-time, 13-week contract
- Monday Friday, 8:00 AM 5:00 PM
- Daily travel within an assigned territory; mileage reimbursement provided
Qualifications:
- Active Physical Therapist (PT) license in good standing for the state of practice
- A minimum of 1 year of recent home health physical therapy (field) experience
- Strong OASIS assessment experience required
- Proficiency with Home Care Home Base (HCHB) preferred
- Excellent clinical judgment, communication, and organizational abilities
- Reliable transportation, valid drivers license, and auto insurance
- Ability to independently manage a flexible, field-based schedule
Primary Responsibilities:
- Conduct comprehensive patient evaluations and OASIS assessments where applicable
- Design and implement individualized therapy care plans targeting functional mobility, strength, balance, and pain management
- Deliver skilled, hands-on interventions such as therapeutic exercises, gait and transfer training, and fall prevention strategies
- Assess progress, update goals, and communicate with physicians and the care team
- Instruct and educate patients and caregivers on exercise routines, safety protocols, and use of assistive devices
- Document care and outcomes promptly using HCHB
- Collaborate with an interdisciplinary team to ensure coordinated, holistic care
- Participate in interdisciplinary meetings and ongoing education as needed
- Adhere to best practices, agency guidelines, and regulatory requirements

Compensation for this position ranges from $28.06 to $60.00. Eligible candidates may also receive healthcare benefits, housing, and meals/incidentals. Full details of our healthcare benefits plan can be found here. This posting is open for 60 days after the posting date.
