Elevate patient care as a Home Health Physical Therapist delivering essential physical therapy services directly to clients in their homes. This 13-week, full-time contract opportunity offers the chance to make a tangible difference in patients strength, safety, and independence while working in a dynamic, field-based setting.

Schedule:

• Monday Friday, 8:00 AM 5:00 PM

Key Qualifications:

• Active and unrestricted Physical Therapist (PT) license in the practicing state
• At least one year of recent home health PT experience preferred; field experience highly valued
• Proficiency with OASIS assessment tools
• Familiarity with Home Care Home Base (HCHB) electronic documentation (preferred)
• Strong functional assessment and clinical decision-making skills
• Ability to independently manage a mobile caseload and communicate effectively
• Reliable transportation, valid driver's license, and auto insurance

Primary Responsibilities:

• Complete comprehensive in-home physical therapy assessments, including OASIS documentation as needed
• Design and execute evidence-based care plans aimed at maximizing patient mobility, safety, and independence
• Deliver skilled therapeutic interventions such as neuromuscular re-education, gait and transfer training, exercise, and fall prevention
• Educate patients and caregivers on tailored exercise programs, safety precautions, and effective use of assistive devices
• Maintain accurate, timely documentation within HCHB
• Collaborate closely with the multidisciplinary care team to coordinate effective patient support
• Attend interdisciplinary meetings and participate in case conferences or ongoing training as required
• Uphold all regulatory, agency, and Medicare guidelines

$2,839 - $3,003/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
