Job Description
Deliver essential in-home care as a compassionate Home Health Physical Therapist, providing high-quality physical therapy services that foster patient independence and improved mobility. In this contract role, youll have the autonomy to manage your caseload while working collaboratively with an interdisciplinary healthcare team to achieve the best outcomes.
Schedule:
- Monday Friday, 8:00 AM 5:00 PM
Key Qualifications:
- Active Physical Therapist license in good standing for the state of practice
- At least 1 year of recent field-based Home Health Physical Therapy experience
- Proficiency in Home Care Home Base (HCHB) preferred
- OASIS assessment experience required
- Strong decision-making, clinical assessment, and time management skills
- Reliable transportation, current drivers license, and valid auto insurance
- Ability to work independently and adapt to a flexible, field-based environment
Primary Responsibilities:
- Conduct thorough patient assessments, including OASIS data collection for Start of Care, Recertification, Resumption, and Discharge visits
- Develop and implement customized treatment plans addressing gait, balance, functional mobility, and patient safety
- Deliver effective interventions (therapeutic exercise, neuromuscular re-education, gait/transfer training, fall prevention)
- Review patient progress, adjust care plans, and maintain communication with physicians and interdisciplinary team
- Educate patients and caregivers on home exercise programs, safety, and use of assistive devices
- Document visits promptly using HCHB
- Attend case conferences and ongoing training, adhering to agency and Medicare guidelines
