An opportunity is available for an experienced, compassionate Home Health Physical Therapist who excels at providing patient-centered care in the comfort of patients homes. In this contract role, you will conduct assessments, develop individualized care plans, and support patients as they work toward improved mobility and independence.

Schedule:

• Full-time, MondayFriday, 8:00 AM5:00 PM
• 13-week contract

Key Responsibilities:

• Complete thorough patient evaluations, including OASIS assessments for relevant visits.
• Develop, implement, and modify individualized therapy goals focused on mobility, functional independence, and safety.
• Deliver skilled interventions such as therapeutic exercises, gait and balance training, neuromuscular re-education, and fall prevention.
• Educate patients and caregivers on exercises, adaptive equipment, and safe mobility strategies.
• Collaborate closely with other members of the interdisciplinary care team to coordinate therapy with other services.
• Maintain accurate, timely documentation using the Home Care Home Base (HCHB) electronic charting platform.
• Attend interdisciplinary meetings and case conferences as needed.
• Comply with regulatory requirements, professional standards, and agency policies.

Qualifications:

• Active Physical Therapist (PT) license in good standing in the state of practice.
• At least 1 year of recent home health physical therapy experience (field experience preferred).
• OASIS assessment experience required; proficiency with Home Care Home Base (HCHB) preferred.
• Excellent clinical assessment, communication, and planning skills.
• Strong organizational abilities and time management skills for an independent, field-based role.
• Reliable transportation, a current drivers license, and up-to-date auto insurance.

$2,727 - $2,885/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
