This contract position involves providing skilled home health physical therapy services, empowering individuals to achieve greater mobility, strength, and independence in familiar surroundings. You will function autonomously as a clinician while remaining closely connected to an interdisciplinary team dedicated to exceptional patient outcomes.
Schedule & Details:
- Full-time, 13-week contract
- Monday Friday, 8:00 AM 5:00 PM
- Caseload: Approximately 2532 points per week (typically 57 visits/day)
- Field-based role visiting patient homes (mileage reimbursement included)
- Utilizes Home Care Home Base (HCHB) for documentation and OASIS assessments
Key Responsibilities:
- Conduct comprehensive evaluations, including required OASIS assessments
- Create and implement individualized care plans to address mobility, strength, pain, and safety
- Provide skilled interventions: therapeutic exercise, neuromuscular re-education, gait/transfer training, and fall prevention
- Monitor and communicate patient progress, updating care plans as needed
- Educate patients and caregivers on exercises, safety, and assistive device use
- Maintain detailed, timely documentation within HCHB
- Collaborate with nurses, therapists, social workers, and aides for coordinated care
- Participate in interdisciplinary meetings and continuing education; ensure adherence to agency and regulatory standards
Desired Qualifications:
- Active Physical Therapist (PT) license in good standing (state of practice)
- At least 1 year of recent home health physical therapy experience
- Required: OASIS experience
- Preferred: Proficiency with Home Care Home Base (HCHB)
- Effective clinical decision-making and patient assessment skills
- Strong communication, time management, and organization abilities
- Reliable vehicle, valid drivers license, and insurance
- Comfort working independently and managing a field-based schedule
