A rewarding opportunity is available for an experienced Home Health Physical Therapist (PT) to provide skilled therapy services within patient residences. This role focuses on enhancing patients' mobility, strength, balance, and functional independence through compassionate, individualized care. Key Responsibilities:

• Perform comprehensive physical therapy evaluations, including OASIS assessments for Start of Care, Resumption, Recertification, and Discharge when applicable.
• Develop and implement tailored treatment plans targeting functional mobility, gait training, pain management, and fall prevention.
• Administer skilled interventions such as therapeutic exercises, neuromuscular re-education, transfer training, and safety assessments.
• Monitor patient progress, adjust treatment plans accordingly, and communicate updates with physicians and interdisciplinary team members.
• Educate patients and caregivers on home exercises, safety techniques, and assistive device usage.
• Maintain accurate, timely electronic documentation using Home Care Home Base (HCHB).
• Collaborate with nurses, occupational therapists, social workers, and other care team members to ensure comprehensive patient care.
• Adhere to agency policies, regulatory standards, and Medicare home health guidelines.

Qualifications:

• Active Physical Therapist license in good standing within the state of practice.
• At least one year of recent home health physical therapy experience; field experience preferred.
• Proficiency with Home Care Home Base (HCHB) and OASIS assessments.
• Strong clinical judgment and functional assessment capabilities.
• Reliable transportation, valid drivers license, and insurance to support daily travel within the assigned territory.
• Excellent communication, time management, and organizational skills.

Schedule & Location:

• Full-time, 13-week contract position.
• Monday through Friday, 8:00 AM 5:00 PM.
• Patient visits typically range from 5 to 7 per day, with a caseload of approximately 2532 points weekly.
• Field-based role requiring daily travel to patient homes in and around Georgetown, KY. Mileage reimbursement provided.
