Seeking an experienced and compassionate clinician to provide skilled physical therapy services in a home health setting. This contract role focuses on supporting patients as they regain strength, mobility, and functional independence. Deliver personalized care while working independently and collaborating with an interdisciplinary team, making a direct impact on patient outcomes.

Schedule & Setting:

• 13-week contract, full-time
• Monday Friday, 8:00 AM 5:00 PM
• Approximately 2532 points per week (averaging 57 patient visits per day)
• Field-based, providing care in patient homes
• Daily travel within an assigned territory (mileage reimbursement available)

Key Responsibilities:

• Conduct comprehensive physical therapy evaluations, including OASIS assessments
• Develop and implement individualized care plans addressing functional mobility, strength, balance, gait, and safety
• Administer skilled interventions: therapeutic exercises, gait training, neuromuscular re-education, transfer training, fall prevention
• Monitor patient progress, adapt plans as needed, and communicate status to physicians and team members
• Educate patients and caregivers on exercises, safety, and home programs
• Complete timely electronic documentation using Home Care Home Base (HCHB)
• Participate in case conferences, interdisciplinary meetings, and ongoing professional development
• Comply with all regulatory standards and agency policies

Qualifications:

• Active and current Physical Therapist (PT) license in good standing
• At least 1 year of recent home health physical therapy experience required
• OASIS assessment experience
• Proficiency with Home Care Home Base (HCHB) preferred
• Reliable transportation, valid drivers license, and auto insurance
• Excellent time management, independence, and clear communication skills

$2,016 - $2,132/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
