An exciting opportunity is available for a dedicated Home Health Physical Therapist (PT) to provide skilled, compassionate care to patients in their own homes. This is a full-time, 13-week contract role, requiring commitment and autonomy to deliver exceptional patient-centered therapy.

Schedule:

• Monday Friday, 8:00 AM 5:00 PM

Key Qualifications:

• Active Physical Therapist (PT) license in good standing in the state of practice
• At least 1 year of recent home health physical therapy experience (field-based experience preferred)
• OASIS assessment experience required
• Familiarity with Home Care Home Base (HCHB) preferred
• Solid clinical assessment and decision-making abilities
• Strong organizational and time management skills
• Reliable transportation, a valid drivers license, and current auto insurance
• Ability to work independently while effectively communicating with the care team

Primary Responsibilities:

• Perform comprehensive in-home physical therapy evaluations, including OASIS assessments as needed
• Develop and execute individualized plans that foster improved mobility, strength, balance, and safety
• Provide skilled interventions (therapeutic exercises, gait and transfer training, fall prevention, pain management, etc.)
• Adjust treatment plans as patient needs change and communicate updates to the healthcare team
• Educate patients and caregivers regarding exercises, safety techniques, and assistive devices
• Complete thorough and timely documentation using Home Care Home Base (HCHB)
• Coordinate with nurses, therapists, and social workers to ensure well-rounded care
• Attend interdisciplinary meetings and case conferences as assigned, maintaining compliance with agency and regulatory standards

$2,299 - $2,432/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
