Bring your clinical expertise and compassion to a key role delivering essential in-home care. This opportunity involves working as a Home Health Physical Therapist, supporting patients in attaining optimal mobility and independence directly in their own homes.

Schedule:

• Full-time, 13-week contract
• Monday Friday, 8:00 AM 5:00 PM
• Daily travel within assigned territory (mileage reimbursement provided)
• Caseload of approximately 2532 points per week (typically 57 patient visits per day, dependent on visit types)

Key Responsibilities:

• Perform comprehensive physical therapy assessments, including OASIS documentation when required
• Develop and implement individualized care plans to improve functional mobility, strength, balance, and safety
• Deliver skilled interventions such as therapeutic exercises, neuromuscular re-education, gait and transfer training, and fall prevention strategies
• Monitor patient progress, adjust care plans as appropriate, and communicate updates to physicians and care teams
• Educate patients and caregivers on exercise regimens, safety protocols, assistive device use, and home exercise programs
• Maintain accurate, timely electronic documentation in Home Care Home Base (HCHB)
• Collaborate with multidisciplinary team members to ensure coordinated, high-quality care
• Attend case conferences, interdisciplinary meetings, and pursue ongoing professional development

Qualifications:

• Active, good-standing Physical Therapist (PT) license in the state of practice
• Minimum one year of recent field-based Home Health Physical Therapy experience
• OASIS assessment experience required
• Proficiency with Home Care Home Base (HCHB) preferred
• Strong decision-making, communication, and time management skills
• Reliable vehicle, valid drivers license, and up-to-date auto insurance
• Ability to work independently and manage a flexible, field-based schedule
