Providing skilled physical therapy services in the home setting, this role supports patients in improving mobility, strength, balance, and overall function. The position requires a self-motivated and organized physical therapist capable of working independently while delivering patient-centered care in Gaithersburg, MD.

Key Responsibilities:

• Perform comprehensive physical therapy evaluations, including OASIS assessments as needed.
• Develop and implement individualized plans of care focusing on mobility, strength, balance, gait, pain management, and safety.
• Deliver skilled interventions such as therapeutic exercise, neuromuscular re-education, gait and transfer training, and fall prevention.
• Monitor patient progress, adjust treatment plans accordingly, and communicate updates to physicians and care teams.
• Educate patients and caregivers on exercises, safe practices, assistive device use, and home exercise maintenance.
• Maintain accurate and timely documentation using Home Care Home Base (HCHB).
• Collaborate with interdisciplinary team members including nurses, occupational therapists, speech-language pathologists, and social workers.
• Participate in case conferences, meetings, and continuing education.
• Comply with agency policies, regulatory requirements, and Medicare home health guidelines.

Qualifications:

• Active Physical Therapist license in good standing for the state of practice.
• Minimum one year of recent home health physical therapy experience; field experience preferred.
• Experienced with OASIS assessments; proficiency with HCHB preferred.
• Strong clinical decision-making and assessment skills.
• Reliable transportation with valid drivers license and auto insurance.
• Ability to manage a flexible, field-based schedule and work independently.
• Excellent communication, organizational, and time management abilities.

Job Details:

• Contract Type: Full-time, 13-week contract.
• Schedule: Monday through Friday, 8:00 AM 5:00 PM.
• Caseload: Approximately 57 patient visits per day (2532 points weekly).
• Setting: Patient homes within assigned territory; mileage reimbursement provided.
• Documentation: Utilize HCHB electronic system.
