Job Description

Provide expert physical therapy care in patients homes as a Home Health Physical Therapist on a 13-week contract. This role is ideal for clinicians who are dedicated to improving patient mobility and independence while working in a supportive, collaborative environment.

Responsibilities:

  • Perform comprehensive assessments evaluating patients physical conditions and mobility limitations
  • Develop and implement individualized treatment plans aligned with patient goals and physician instructions
  • Administer therapeutic exercises, manual therapy, and modalities to enhance strength and functional abilities
  • Educate patients and caregivers on proper techniques for exercises, transfers, and assistive devices
  • Monitor patient progress and adjust treatment plans as needed for optimal outcomes
  • Document all care accurately in electronic medical records, adhering to regulatory guidelines
  • Coordinate with interdisciplinary team members to ensure cohesive care and communication
  • Maintain compliance with professional ethical standards and state/federal regulations

Qualifications:

  • Bachelors or Masters degree in Physical Therapy from an accredited program
  • Current state license or eligibility for Physical Therapist licensure
  • Experience providing care in home health settings preferred
  • Proficiency with HomeCare HomeBase (HCHB) electronic medical record software
  • Strong clinical reasoning and critical thinking abilities
  • Excellent interpersonal and communication skills with a compassionate, patient-centered approach
  • Familiarity with Medicare documentation and regulatory requirements for home health
  • Valid drivers license and reliable transportation for home visits
