Deliver vital rehabilitation care with autonomy as a Home Health Physical Therapist. This full-time, 13-week contract role places you at the forefront of patient recovery, working directly in patients homes to restore mobility, build strength, and promote safe, independent living. Youll be trusted to complete detailed assessments, develop personalized treatment plans, and deliver skilled interventionsa meaningful opportunity for therapists passionate about improving lives through evidence-based care.

Schedule:

• Monday through Friday, 8:00 AM 5:00 PM
• Expect 57 patient visits per day, totaling about 2532 points weekly

Essential Responsibilities:

• Conduct thorough physical therapy evaluations, including OASIS assessments at pertinent episodes.
• Design and implement individualized plans targeting mobility, strength, pain, fall prevention, and functional progression.
• Deliver in-home skilled therapies: therapeutic exercise, transfer and gait training, neuromuscular re-education, and safety instruction.
• Monitor progress, adjusting interventions and collaborating closely with physicians and interdisciplinary team members.
• Provide caregiver and patient education on exercise programs, assistive devices, and safety strategies.
• Maintain accurate, timely electronic records through Home Care Home Base (HCHB).
• Participate in interdisciplinary meetings and contribute actively to patient care plans.

Desired Experience and Qualifications:

• Active PT license in good standing for the state
• At least 1 year of recent field-based Home Health PT experience
• Experience with HCHB (preferred) and OASIS (required)
• Demonstrated clinical acumen and strong assessment skills
• Effective communicator, self-starter, and highly organized
• Reliable transportation, valid drivers license, and current auto insurance
