A dedicated and compassionate professional is needed for a contract position providing physical therapy services to patients within their homes. This role centers on advancing patients mobility, strength, and overall independence through individualized, high-quality care.

Schedule:

• Monday to Friday, 8:00 AM 5:00 PM

Key Qualifications:

• Current, active PT license in good standing
• Minimum 1 year of recent Home Health Physical Therapy experience
• OASIS assessment experience required
• Proficient with Home Care Home Base (HCHB) preferred
• Demonstrated ability to work independently and manage a flexible field-based caseload
• Effective communication, time management, and organizational skills
• Reliable transportation, valid driver's license, and auto insurance

Responsibilities:

• Conduct comprehensive physical therapy evaluations (including OASIS as required)
• Develop and implement personalized care plans to address functional mobility, gait, strength, balance, safety, and pain management
• Deliver skilled interventions including therapeutic exercises, gait training, neuromuscular re-education, and fall prevention
• Assess progress, revise treatment plans as needed, and communicate updates to the interdisciplinary team
• Educate patients and caregivers on exercises, equipment, safety, and home exercise plans
• Maintain accurate and timely documentation within the Home Care Home Base (HCHB) system
• Collaborate with nurses, occupational therapists, speech therapists, social workers, and home health aides
• Participate in case conferences, interdisciplinary meetings, and ongoing professional development
• Ensure compliance with agency policies and regulatory standards
