Schedule:
- Monday Friday, 8:00 AM 5:00 PM
Key Qualifications:
- Active Physical Therapist (PT) license in good standing
- Minimum 1 year of recent Home Health Physical Therapy experience
- OASIS assessment experience required
- Proficiency with Home Care Home Base (HCHB) preferred
- Reliable transportation, valid drivers license, and auto insurance
- Strong clinical, organizational, and time management abilities
- Ability to work independently in a field-based setting
Primary Responsibilities:
- Deliver comprehensive patient assessments, including OASIS, as needed
- Develop and implement personalized treatment plans focusing on mobility, strength, balance, and safety
- Provide evidence-based therapeutic interventions (therapeutic exercise, gait and transfer training, fall prevention, etc.)
- Educate patients and caregivers on home exercise programs, assistive devices, and strategies for safe mobility
- Document all care activities and progress in Home Care Home Base (HCHB)
- Collaborate with interdisciplinary team members to coordinate holistic patient care
- Participate in case conferences and continuing education opportunities
