Job Description
A dedicated and compassionate professional is needed for a contract position providing physical therapy services to patients within their homes. This role centers on advancing patients mobility, strength, and overall independence through individualized, high-quality care.
Schedule:
- Monday to Friday, 8:00 AM 5:00 PM
Key Qualifications:
- Current, active PT license in good standing
- Minimum 1 year of recent Home Health Physical Therapy experience
- OASIS assessment experience required
- Proficient with Home Care Home Base (HCHB) preferred
- Demonstrated ability to work independently and manage a flexible field-based caseload
- Effective communication, time management, and organizational skills
- Reliable transportation, valid driver's license, and auto insurance
Responsibilities:
- Conduct comprehensive physical therapy evaluations (including OASIS as required)
- Develop and implement personalized care plans to address functional mobility, gait, strength, balance, safety, and pain management
- Deliver skilled interventions including therapeutic exercises, gait training, neuromuscular re-education, and fall prevention
- Assess progress, revise treatment plans as needed, and communicate updates to the interdisciplinary team
- Educate patients and caregivers on exercises, equipment, safety, and home exercise plans
- Maintain accurate and timely documentation within the Home Care Home Base (HCHB) system
- Collaborate with nurses, occupational therapists, speech therapists, social workers, and home health aides
- Participate in case conferences, interdisciplinary meetings, and ongoing professional development
- Ensure compliance with agency policies and regulatory standards
