Job Description

Make a meaningful impact helping individuals regain their independence as a Home Health Physical Therapist. This contract opportunity allows you to provide essential physical therapy services directly to patients in their homes, focusing on improving mobility, strength, and overall safety. Youll complete thorough evaluations, develop tailored treatment plans, and collaborate closely with the healthcare team to promote optimal outcomes.

Schedule:

  • Full-time, 13-week contract
  • Monday Friday, 8:00 AM 5:00 PM
  • Approximately 2532 points per week (about 57 patient visits daily)
  • Field-based in patient homes, with mileage reimbursement for travel within your assigned territory

Key Responsibilities:

  • Conduct comprehensive evaluations using OASIS for Start of Care, Resumption, Recertification, and Discharge
  • Design and implement individualized therapy plans focusing on mobility, strength, balance, gait, and pain management
  • Deliver skilled interventions, including therapeutic exercise, gait and transfer training, neuromuscular re-education, and fall prevention strategies
  • Educate patients and caregivers on exercise, safety, and the use of assistive devices
  • Maintain accurate documentation within Home Care Home Base (HCHB)
  • Coordinate with nurses, therapists, social workers, and aides to ensure cohesive care
  • Participate in interdisciplinary meetings and meet regulatory and agency standards

Desired Skills & Experience:

  • Active Physical Therapist license in good standing in the state of practice
  • Minimum 1 year of recent field-based home health physical therapy experience
  • OASIS assessment experience required
  • Proficiency with HCHB preferred
  • Reliable transportation, valid drivers license, and insurance
  • Excellent clinical, communication, and organizational skills
  • Ability to work autonomously and manage a flexible field schedule
