Physical therapists with a passion for home health are invited to embark on a rewarding 13-week contract supporting patients with mobility, strength, and independence in a field-based setting. Deliver high-quality, patient-centered care with the autonomy of managing your caseload and schedule, while collaborating closely with an experienced interdisciplinary team.

Schedule & Setting:

• Monday Friday, 8:00 AM 5:00 PM
• Full-time, 13-week contract
• Approximately 2532 points per week (typically 57 patient visits per day)
• Deliver services in patients homes
• Daily travel within assigned territory (mileage reimbursement provided)
• Utilize Home Care Home Base (HCHB) for electronic documentation
• OASIS assessments required for various care milestones

Qualifications:

• Active Physical Therapist (PT) license in good standing
• At least 1 year of recent home health PT experience (field experience preferred)
• OASIS assessment experience
• Proficiency in Home Care Home Base (HCHB) preferred
• Reliable transportation, valid drivers license, and current auto insurance
• Strong clinical and functional assessment skills
• Excellent organization, time management, and communication abilities
• Comfortable working independently

Key Responsibilities:

• Perform comprehensive PT evaluations (including OASIS) and establish individualized treatment plans
• Implement skilled interventions: therapeutic exercise, gait and transfer training, neuromuscular re-education, and fall prevention
• Educate patients and caregivers on safety, use of assistive devices, and individualized exercise programs
• Track and modify treatments based on progress; communicate updates to physicians and team
• Maintain accurate, timely documentation in HCHB
• Collaborate with nursing, OT, speech, social work, and home health aides
• Participate in case conferences and ongoing professional development

$2,253 - $2,383/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
