Job Description
A skilled and compassionate Home Health Physical Therapist is needed to provide high-quality care to patients in their homes, supporting improved mobility, independence, and overall wellness. This contract position focuses on patient-centered treatment and collaboration with a multidisciplinary team.
Schedule/Hours:
- Monday Friday, 8:00 AM 5:00 PM
- Contract length: 13 weeks
- Field-based, with daily travel to patient homes (mileage reimbursement included)
- Caseload: 2532 points weekly (approximately 57 patient visits per day, depending on visit types)
Key Responsibilities:
- Conduct comprehensive in-home physical therapy evaluations, including OASIS assessments (Start of Care, Resumption, Recertification, and Discharge, as needed)
- Create and execute individualized care plans focused on functional mobility, strength, balance, gait, pain management, and safety
- Deliver skilled interventions: therapeutic exercise, gait/transfer training, neuromuscular re-education, and fall prevention strategies
- Reassess and modify treatment plans as patient goals evolve, collaborating with physicians and interdisciplinary healthcare team members
- Educate patients and caregivers on exercises, safety, assistive device use, and home exercise programs
- Maintain detailed, timely documentation using Home Care Home Base (HCHB)
- Participate in case conferences, team meetings, and professional development activities
- Ensure compliance with all regulatory guidelines and agency policies
Qualifications:
- Active Physical Therapist (PT) license in the state of practice (in good standing)
- Minimum one year of recent home health PT experience required
- Proficiency with Home Care Home Base (HCHB) preferred
- OASIS experience required
- Excellent clinical assessment, communication, and organizational skills
- Reliable transportation, valid drivers license, and auto insurance
