Job Description

A skilled and compassionate Home Health Physical Therapist is needed to provide high-quality care to patients in their homes, supporting improved mobility, independence, and overall wellness. This contract position focuses on patient-centered treatment and collaboration with a multidisciplinary team.

Schedule/Hours:

• Monday Friday, 8:00 AM 5:00 PM
• Contract length: 13 weeks
• Field-based, with daily travel to patient homes (mileage reimbursement included)
• Caseload: 2532 points weekly (approximately 57 patient visits per day, depending on visit types)

Key Responsibilities:

• Conduct comprehensive in-home physical therapy evaluations, including OASIS assessments (Start of Care, Resumption, Recertification, and Discharge, as needed)
• Create and execute individualized care plans focused on functional mobility, strength, balance, gait, pain management, and safety
• Deliver skilled interventions: therapeutic exercise, gait/transfer training, neuromuscular re-education, and fall prevention strategies
• Reassess and modify treatment plans as patient goals evolve, collaborating with physicians and interdisciplinary healthcare team members
• Educate patients and caregivers on exercises, safety, assistive device use, and home exercise programs
• Maintain detailed, timely documentation using Home Care Home Base (HCHB)
• Participate in case conferences, team meetings, and professional development activities
• Ensure compliance with all regulatory guidelines and agency policies

Qualifications:

• Active Physical Therapist (PT) license in the state of practice (in good standing)
• Minimum one year of recent home health PT experience required
• Proficiency with Home Care Home Base (HCHB) preferred
• OASIS experience required
• Excellent clinical assessment, communication, and organizational skills
• Reliable transportation, valid drivers license, and auto insurance
