Schedule:
- Monday to Friday, 8:00 AM 5:00 PM
Key Responsibilities:
- Conduct thorough physical therapy evaluations, including OASIS assessments
- Create and implement individualized treatment plans targeting mobility, strength, balance, and safety
- Deliver skilled interventions such as therapeutic exercise, gait training, and fall prevention
- Monitor patient progress, adjusting care plans as necessary and communicating with healthcare team members
- Educate patients and caregivers on exercises, safety, use of assistive devices, and home therapy programs
- Accurately document all care in the Home Care Home Base (HCHB) system
- Collaborate with nurses, therapists, aides, and other professionals to ensure coordinated care
- Attend required case conferences, meetings, and participate in ongoing education
Qualifications:
- Active, unrestricted Physical Therapist license in the relevant state
- At least 1 year of recent home health PT experience (including field-based care)
- Proficient with OASIS; Home Care Home Base (HCHB) experience preferred
- Reliable transportation, valid drivers license, and current auto insurance
- Excellent organizational, clinical decision-making, and communication skills
- Ability to independently manage a flexible home health schedule
