Provide expert physical therapy care in patients homes as a Home Health Physical Therapist (PT) within a supportive and flexible environment. This contract position is full-time and offers the opportunity to deliver individualized rehabilitation services that enhance patients mobility, strength, balance, and independence.

Responsibilities:

• Perform comprehensive physical therapy evaluations, including OASIS assessments for Start of Care, Resumption, Recertification, and Discharge as applicable.
• Develop and implement personalized plans of care focused on functional mobility, pain management, gait training, neuromuscular re-education, transfer training, and fall prevention.
• Deliver skilled interventions and monitor patient progress, adjusting treatment plans accordingly.
• Educate patients and caregivers on exercise programs, safety practices, and assistive device use.
• Document treatments accurately and timely using Home Care Home Base (HCHB) electronic system.
• Collaborate with interdisciplinary teams including nurses, occupational therapists, speech-language pathologists, social workers, and home health aides to ensure coordinated care.
• Participate in case conferences and adhere to all regulatory, Medicare, and agency policies.

Qualifications:

• Active and valid Physical Therapist license in the relevant state.
• Minimum 1 year of recent home health physical therapy experience; field experience preferred.
• Proficiency with Home Care Home Base (HCHB) and OASIS assessments.
• Strong clinical reasoning, organizational and communication skills.
• Reliable transportation, valid drivers license, and auto insurance.
• Ability to work independently with a flexible, field-based schedule.

Schedule & Contract Details:

• Full-time, 13-week contract
• Monday Friday, 8:00 AM 5:00 PM
• Caseload of approximately 2532 points per week (57 patient visits per day)
• Daily travel within assigned territory with mileage reimbursement
