Provide expert physical therapy care as a Home Health Physical Therapist (PT), delivering personalized treatment to patients in their residences within the Florence, SC area. This contract position involves improving patients mobility, strength, balance, and overall independence through skilled interventions and compassionate support.

Key Responsibilities:

• Conduct thorough physical therapy evaluations, including OASIS assessments for Start of Care, Resumption, Recertification, and Discharge as applicable.
• Develop and implement individualized plans of care targeting functional mobility, pain management, gait training, transfer techniques, and fall prevention.
• Provide therapeutic exercises, neuromuscular re-education, and patient education on safety, assistive device use, and home exercise programs.
• Monitor patient progress, adjust treatment plans accordingly, and communicate changes to physicians and interdisciplinary team members.
• Maintain accurate and timely documentation using Home Care Home Base (HCHB).
• Collaborate with nurses, occupational therapists, speech-language pathologists, social workers, and home health aides to coordinate care.
• Adhere to agency policies and Medicare home health regulations.

Qualifications:

• Active Physical Therapist (PT) license in good standing within the state of practice.
• Minimum 1 year of recent home health physical therapy experience; field experience preferred.
• Proficiency with HCHB documentation system preferred.
• Experience with OASIS assessments required.
• Strong clinical decision-making and functional assessment skills.
• Reliable transportation, valid drivers license, and auto insurance.
• Ability to work independently with a flexible, patient-focused schedule.
• Excellent communication, organizational, and time management skills.

Schedule & Details:

• Full-time, 13-week contract
• Monday through Friday, 8:00 AM 5:00 PM
• Caseload of approximately 2532 points weekly (57 patient visits daily)
• Mileage reimbursement provided for patient home visits within assigned territory
