Job Description
Deliver impactful care to patients in their homes as an experienced Home Health Physical Therapist. This contract opportunity offers a rewarding environment for clinically skilled, compassionate professionals dedicated to making a difference in the lives of others through one-on-one therapy and functional support.
Schedule:
- Full-time, 13-week contract
- Monday Friday, 8:00 AM 5:00 PM
- Approximately 2532 points per week (typical caseload of 57 patient visits daily)
- Daily travel required within an assigned territory; mileage reimbursement provided
Responsibilities:
- Complete comprehensive physical therapy evaluations, including OASIS assessments when applicable
- Create and carry out individualized care plans to address mobility, strength, balance, pain management, safety, and functional goals
- Deliver skilled interventions such as therapeutic exercise, neuromuscular re-education, gait and transfer training, and fall prevention
- Monitor patient progress, update care plans, and collaborate with physicians and interdisciplinary healthcare team members
- Educate patients and caregivers on rehabilitation techniques, home safety, use of assistive devices, and self-management strategies
- Maintain accurate and timely documentation in Home Care Home Base (HCHB)
- Participate in case conferences and team meetings
- Ensure compliance with all regulatory and Medicare home health requirements
Qualifications:
- Active Physical Therapist license in good standing
- Minimum 1 year of recent field-based Home Health PT experience
- OASIS assessment experience required
- Proficiency with Home Care Home Base (HCHB) preferred
- Strong clinical judgment and organizational skills
- Reliable transportation, valid drivers license, and auto insurance
