This contract opportunity offers you the chance to utilize your clinical expertise while helping patients achieve greater independence in the comfort of their own homes.

Schedule:

  • Monday Friday, 8:00 AM 5:00 PM
  • Full-time, 13-week contract

Your expertise will support a caseload of approximately 2532 points per week (typically 57 home visits per day). Youll perform comprehensive initial and follow-up assessments (including OASIS), create individualized care plans, and deliver effective PT interventions for a variety of functional needsfrom gait training and pain management, to balance, transfer training, and fall prevention. Success in this role requires strong documentation skills in Home Care Home Base (HCHB) and effective interdisciplinary collaboration to drive excellent outcomes.

Your responsibilities will include:

  • Conducting detailed physical therapy assessments and implementing tailored treatment plans
  • Utilizing OASIS for Start of Care, Resumption, Recertification, and Discharge (as needed)
  • Delivering evidence-based interventions and patient/family education on home exercise and safety
  • Evaluating patient progress and updating care plans in coordination with physicians and other team members
  • Maintaining accurate, timely documentation in HCHB
  • Regular participation in case conferences and ongoing education
  • Ensuring compliance with agency standards and Medicare regulations

Qualifications:

  • Active PT license (in good standing) in the state of practice
  • Minimum 1 year of recent home health PT experience (field experience preferred)
  • OASIS and HCHB proficiency (required and highly preferred, respectively)
  • Reliable vehicle, drivers license, and auto insurance
  • Strong communication, time management, and organizational abilities

$1,609 - $1,702/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
