Job Description

Deliver high-quality, patient-centered care as a Home Health Physical Therapist, providing skilled services to clients in their homes and supporting recovery, independence, and improved quality of life. This is a contract position that offers a rewarding blend of autonomy and team collaboration.

Schedule:

• Monday Friday, 8:00 AM 5:00 PM
• Full-time, 13-week contract

Key Responsibilities:

• Complete comprehensive physical therapy evaluations in the home setting, including OASIS assessments for start of care, recertification, resumption, and discharge as needed
• Develop and implement individualized treatment plans, focusing on functional mobility, gait, balance, and safe independence
• Deliver skilled interventions such as therapeutic exercise, neuromuscular re-education, transfer training, gait training, pain management, and fall prevention
• Monitor progress, adjust treatment plans as indicated, and promptly communicate changes to physicians and clinical team
• Educate patients and caregivers on safe mobility, use of assistive devices, and home exercises
• Accurately document care in Home Care Home Base (HCHB)
• Collaborate with the interdisciplinary care team for coordinated, patient-focused services
• Participate in case conferences, team meetings, and continuing education activities
• Adhere to agency protocols, regulatory guidelines, and Medicare standards

Required Qualifications:

• Active Physical Therapist license in good standing (state of practice)
• At least 1 year of recent home health physical therapy experience in the field
• Experience with Home Care Home Base (HCHB) and OASIS assessments
• Strong clinical assessment and decision-making abilities
• Effective communication and organization skills
• Reliable transportation, valid drivers license, and auto insurance
• Comfortable working independently in a field-based role

$2,839 - $3,003/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
