Make a direct impact on the lives of patients recovering at home by providing high-quality, compassionate care as a Home Health Physical Therapist. This full-time, 13-week contract role offers the unique opportunity to deliver individualized therapy services in the comfort and familiarity of each patients residence.

Schedule:

• Monday Friday, 8:00 AM 5:00 PM

Key Qualifications:

• Current, unrestricted Physical Therapist (PT) license in the state of practice
• At least 1 year of recent home health physical therapy experience (field-based background highly valued)
• OASIS assessment experience required
• Proficiency with Home Care Home Base (HCHB) preferred
• Strong functional assessment, clinical decision-making, and documentation skills
• Excellent time management, organization, and communication abilities
• Reliable transportation, valid driver's license, and proof of auto insurance
• Comfortable working independently with a flexible field-based schedule

Core Responsibilities:

• Conduct thorough in-home physical therapy evaluations using OASIS for appropriate visits
• Develop and implement tailored care plans focused on mobility, strength, pain management, safety, and patient goals
• Administer therapeutic exercises, transfer and gait training, neuromuscular re-education, and fall prevention strategies
• Educate patients and caregivers on exercises, safe device use, and home programs to encourage independence
• Maintain timely and accurate documentation in HCHB
• Collaborate with other healthcare professionals to ensure an integrated care approach
• Participate in interdisciplinary meetings, continued education, and case conferences as needed
• Uphold all agency, regulatory, and Medicare standards
