Job Description

Schedule:

  • Full-time, 13-week contract
  • Monday to Friday, 8:00 AM 5:00 PM

Key Responsibilities:

  • Complete comprehensive evaluations, including OASIS assessments for Start of Care, Resumption, Recertification, and Discharge as applicable.
  • Develop tailored therapy plans focused on mobility, strength, pain reduction, gait, and safety.
  • Deliver skilled interventions such as therapeutic exercises, neuromuscular re-education, transfer and gait training, and fall prevention strategies.
  • Regularly assess progress, adapt treatment plans, and communicate updates to physicians and interdisciplinary teams.
  • Teach patients and caregivers safe exercise techniques, strategies for daily living, and use of assistive devices.
  • Accurately document all care and progress using Home Care Home Base (HCHB).
  • Collaborate closely with other healthcare professionals at interdisciplinary meetings and participate in case conferences as needed.
  • Maintain compliance with agency policies, Medicare guidelines, and regulatory standards.

Preferred Qualifications:

  • Active and unrestricted Physical Therapist (PT) license in Washington.
  • At least 1 year of recent experience in home health physical therapy.
  • Experience with Home Care Home Base (HCHB) and OASIS documentation.
  • Strong assessment and clinical decision-making abilities.
  • Reliable transportation, valid drivers license, and auto insurance.
  • Excellent organization, communication, and time management skills.
