An opportunity for an experienced Home Health Physical Therapist (PT) is available to provide skilled, patient-centered care in the comfort of patients homes. This contract position requires delivering individualized physical therapy services to help patients improve mobility, strength, balance, and overall functional independence. The ideal candidate is proactive, organized, and comfortable working independently while maintaining collaboration with an interdisciplinary healthcare team.

Key Responsibilities:

• Perform comprehensive physical therapy evaluations including OASIS assessments (Start of Care, Resumption, Recertification, Discharge).
• Develop and implement customized treatment plans addressing functional mobility, strength, gait, pain management, transfer training, and fall prevention.
• Deliver skilled interventions such as therapeutic exercises, neuromuscular re-education, and gait training.
• Monitor patient progress, adjust plans as necessary, and communicate updates with physicians and team members.
• Educate patients and caregivers on exercise techniques, safety, use of assistive devices, and home programs.
• Maintain accurate and timely documentation using Home Care Home Base (HCHB).
• Collaborate closely with nurses, occupational therapists, speech-language pathologists, social workers, and home health aides to coordinate care.
• Adhere to agency policies, Medicare guidelines, and regulatory standards.

Qualifications:

• Active Physical Therapist license in good standing.
• Minimum 1 year of recent home health physical therapy experience; field experience preferred.
• Experience with OASIS assessments required.
• Proficiency with Home Care Home Base (HCHB) preferred.
• Reliable transportation, valid drivers license, and auto insurance.
• Strong clinical decision-making, communication, and organizational skills.
• Ability to work independently and manage a flexible, field-based schedule.

Position Details:

• Location: Eustis, FL.
• Schedule: Monday through Friday, 8:00 AM 5:00 PM.
• Caseload: Approximately 2532 points/week (57 patient visits per day).
• Contract Length: Full-time, 13-week contract.
• Travel: Daily travel within assigned territory; mileage reimbursement provided.
