Bring your expertise and compassion to patients homes as a Home Health Physical Therapist. In this highly independent role, youll make a meaningful difference by helping individuals regain mobility, strength, and confidence while remaining in the comfort of their own environments. You will collaborate closely with an interdisciplinary team, create personalized care plans, and directly support patients journeys toward greater functional independence.

Schedule & Job Details:

• Monday Friday, 8:00 AM 5:00 PM
• Full-time, 13-week contract assignment
• Expect a caseload of approximately 2532 points weekly (generally 57 visits per day, visit-type dependent)
• Field-based role with daily travel to patient homes (mileage reimbursement provided)
• Utilize Home Care Home Base (HCHB) for documentation and OASIS for required assessments

Qualifications:

• Current Physical Therapist (PT) license in good standing
• At least 1 year of recent home health PT experience
• Experience with OASIS assessments and electronic documentation systems (preferably HCHB)
• Strong clinical decision-making, assessment, and organizational skills
• Reliable transportation, valid drivers license, and auto insurance
• Ability to work autonomously and manage a flexible, field-based schedule
• Excellent communication and interpersonal skills

Key Responsibilities:

• Complete comprehensive assessments, including OASIS when required
• Develop, implement, and update individualized care plans targeting patients functional goals
• Deliver skilled therapy interventionstherapeutic exercise, gait and balance training, pain management, safety education, and fall prevention
• Educate patients and caregivers on exercises, mobility aids, safety precautions, and home programs
• Maintain thorough documentation and comply with regulatory standards
• Participate actively in interdisciplinary meetings and case conferences

$2,011 - $2,127/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
