Bring your expertise as a Home Health Physical Therapist and make a meaningful impact by guiding patients toward greater independence within their own homes. In this contract role, youll utilize your skillset to deliver personalized, high-quality care, developing and implementing therapy plans that restore function, mobility, and confidence for each patient.

Schedule:

• Monday Friday, 8:00 AM 5:00 PM
• Full-time, 13-week contract
• Field-based visits, averaging 57 home patient visits per day

Key Responsibilities:

• Conduct comprehensive physical therapy evaluations and OASIS assessments as required
• Create and carry out individualized care plans to address functional mobility, balance, gait, pain management, and patient safety
• Deliver skilled therapeutic interventions including neuromuscular re-education, gait/transfer training, and fall prevention
• Routinely assess progress, adapt treatment approaches, and communicate updates with the interdisciplinary healthcare team
• Provide education to patients and caregivers on exercises, safety, and equipment use
• Ensure timely, accurate documentation using Home Care Home Base (HCHB)
• Foster effective collaboration with RNs, OTs, SLPs, medical social workers, and aides
• Participate in interdisciplinary meetings, continuing education, and case conferences

Qualifications:

• Active Physical Therapist (PT) license in good standing in the relevant state
• At least 1 year of recent home health PT experience
• Proficiency with HCHB preferred; OASIS experience required
• Proven clinical assessment and decision-making abilities
• Reliable transportation, current drivers license, and insurance
• Strong organization, communication, and time management skills
• Self-motivated, able to thrive independently in a field-based role
