Job Description

Step into a rewarding role providing vital in-home physical therapy services to patients who need personalized, patient-centered care to regain mobility, strength, and independence. This full-time, 13-week contract position is ideal for an experienced Physical Therapist seeking a field-based opportunity to deliver high-quality therapy in the comfort and privacy of patients' homes.

Schedule:

  • Monday Friday, 8:00 AM 5:00 PM

Core Responsibilities:

  • Conduct thorough physical therapy evaluations, including OASIS assessments for Start of Care, Resumption, Recertification, and Discharge as needed.
  • Develop and implement individualized treatment plans targeting mobility, strength, balance, gait, pain management, and safety.
  • Employ skilled interventions such as therapeutic exercises, neuromuscular re-education, gait and transfer training, and fall prevention strategies.
  • Monitor patient progress and adjust care plans accordingly, communicating updates to physicians and interdisciplinary team members.
  • Provide education to patients and caregivers regarding exercises, safety protocols, assistive devices, and home exercise regimens.
  • Maintain accurate and timely clinical documentation in the Home Care Home Base (HCHB) electronic system.
  • Collaborate with other healthcare professionalsincluding nurses, occupational and speech therapists, medical social workers, and home health aidesto ensure coordinated, comprehensive care.

Qualifications:

  • Active and in-good-standing Physical Therapist license in North Carolina
  • At least 1 year of recent home health physical therapy experience required; strong preference for field-based experience
  • Proficient with OASIS assessments and documentation; experience with HCHB strongly preferred
  • Demonstrated clinical decision-making and assessment skills
  • Reliable transportation, a valid drivers license, and current auto insurance
  • Able to work independently while effectively managing a flexible, mobile caseload
  • Strong communication, organizational, and time management skills
