Job Description
Deliver exceptional patient-centered care by providing skilled physical therapy services directly in patients homes. As a Home Health Physical Therapist, you will play a vital role in helping individuals regain mobility, improve strength and balance, and achieve greater independence through comprehensive treatment plans designed for every patients unique needs.
Schedule and Assignment Details:
- Full-time, 13-week contract
- Monday Friday, 8:00 AM 5:00 PM
- Approximately 2532 points per week (typically 57 patient visits per day)
- Daily travel within assigned territory (mileage reimbursement included)
- Field-based, providing care in patients homes
Key Responsibilities:
- Complete thorough physical therapy evaluations, including OASIS assessments as required
- Develop and implement individualized care plans focused on mobility, strength, gait training, balance, pain management, and fall prevention
- Deliver therapeutic exercises, neuromuscular re-education, transfer training, and safety education
- Monitor and reassess patient progress, modifying care plans as needed for optimal outcomes
- Educate patients and caregivers on exercises, assistive device safety, and home exercise routines
- Maintain timely, accurate documentation using Home Care Home Base (HCHB)
- Collaborate effectively with nurses, OTs, SLPs, MSWs, and aides to ensure coordinated care delivery
- Participate in interdisciplinary meetings and ongoing professional development
Required Qualifications:
- Active Physical Therapist (PT) license in good standing for the state
- Minimum of 1 year of recent home health physical therapy experience (field experience preferred)
- Hands-on experience with OASIS and proficiency in HCHB preferred
- Excellent clinical judgment, time management, and organizational abilities
- Reliable transportation, valid drivers license, and auto insurance required
- Strong communication and self-management skills
