Deliver skilled and compassionate in-home care as a Physical Therapist specializing in home health. Provide essential physical therapy services to adult patients, empowering them to achieve greater mobility, strength, and independence in their everyday lives. This patient-facing field role is ideal for candidates who are self-directed, detail-oriented, and passionate about individualized care.

Schedule:

• Monday Friday, 8:00 AM 5:00 PM
• Full-time, 13-week contract

Key Responsibilities:

• Conduct comprehensive patient evaluations, including OASIS assessments as needed.
• Design and implement tailored physical therapy plans focusing on mobility, balance, strength, and safety.
• Provide skilled interventions such as therapeutic exercises, gait and transfer training, neuromuscular re-education, and fall prevention strategies.
• Track patient progress, adjust plans accordingly, and communicate updates to physicians and interprofessional teammates.
• Educate patients and caregivers regarding exercises, home safety, adaptive equipment, and assistive devices.
• Document care activities promptly and accurately in Home Care Home Base (HCHB).
• Participate actively in interdisciplinary collaboration, care conferences, and ongoing education.
• Ensure compliance with Medicare guidelines and agency policies.
• Travel daily to patient homes within your assigned territory (mileage reimbursed).

Qualifications:

• Current, active Physical Therapist (PT) license in good standing in South Carolina.
• At least 1 year of recent field-based home health physical therapy experience.
• Familiarity with Home Care Home Base (HCHB) preferred.
• OASIS proficiency required.
• Excellent assessment, problem-solving, and clinical decision-making skills.
• Reliable vehicle, valid drivers license, and proof of auto insurance.
• Strong organizational and communication abilities; comfortable working independently.
