Job Description

Deliver exceptional patient-centered care by providing skilled physical therapy services directly in patients homes. As a Home Health Physical Therapist, you will play a vital role in helping individuals regain mobility, improve strength and balance, and achieve greater independence through comprehensive treatment plans designed for every patients unique needs.

Schedule and Assignment Details:

  • Full-time, 13-week contract
  • Monday Friday, 8:00 AM 5:00 PM
  • Approximately 2532 points per week (typically 57 patient visits per day)
  • Daily travel within assigned territory (mileage reimbursement included)
  • Field-based, providing care in patients homes

Key Responsibilities:

  • Complete thorough physical therapy evaluations, including OASIS assessments as required
  • Develop and implement individualized care plans focused on mobility, strength, gait training, balance, pain management, and fall prevention
  • Deliver therapeutic exercises, neuromuscular re-education, transfer training, and safety education
  • Monitor and reassess patient progress, modifying care plans as needed for optimal outcomes
  • Educate patients and caregivers on exercises, assistive device safety, and home exercise routines
  • Maintain timely, accurate documentation using Home Care Home Base (HCHB)
  • Collaborate effectively with nurses, OTs, SLPs, MSWs, and aides to ensure coordinated care delivery
  • Participate in interdisciplinary meetings and ongoing professional development

Required Qualifications:

  • Active Physical Therapist (PT) license in good standing for the state
  • Minimum of 1 year of recent home health physical therapy experience (field experience preferred)
  • Hands-on experience with OASIS and proficiency in HCHB preferred
  • Excellent clinical judgment, time management, and organizational abilities
  • Reliable transportation, valid drivers license, and auto insurance required
  • Strong communication and self-management skills
