Job Description
Experienced physical therapy professionals are invited to impact patient lives through expert care in a home-based setting. This full-time, 13-week engagement focuses on delivering personalized rehabilitation services, supporting patients in regaining independence and optimizing their functional abilities.
Schedule:
- Monday through Friday, 8:00 AM 5:00 PM
What Youll Do:
- Conduct detailed physical therapy evaluations, including OASIS assessments where required.
- Design and implement individualized plans of care aimed at improving mobility, strength, gait, balance, and safety.
- Deliver skilled interventions: therapeutic exercise, neuromuscular re-education, gait and transfer training, and fall prevention strategies.
- Monitor clinical progress, regularly revising care plans to maximize outcomes.
- Educate and train patients and caregivers on safe practices, assistive devices, and custom home exercise routines.
- Accurately document visits and patient updates in Home Care Home Base (HCHB).
- Work in close collaboration with fellow healthcare team members for coordinated, high-quality care.
- Attend relevant meetings, case conferences, and continuing education sessions.
- Uphold compliance with agency protocols and Medicare home health guidelines.
Desired Qualifications:
- Current, active Physical Therapist license (in good standing) in the practicing state
- At least one year of recent home health physical therapy experience
- Familiarity with HCHB (preferred) and proficiency in OASIS documentation (required)
- Confident clinical assessment and decision-making abilities
- Reliable transportation, current drivers license, and car insurance
- Self-directed, organized, and adaptable to a flexible, field-based schedule
- Excellent communication and interpersonal skills
