Job Description

Deliver vital, hands-on patient care as a Home Health Physical Therapist, providing one-on-one therapy to patients in the comfort of their homes. This full-time, 13-week contract position offers a fulfilling blend of clinical independence, patient-centered care, and interprofessional collaborationall while working weekdays.

Schedule & Setting:

• Monday Friday, 8:00 AM 5:00 PM
• Approximately 2532 points per week (57 patient visits per day based on visit types)
• Field-based: travel to patient residences within assigned territory

Key Responsibilities:

• Complete comprehensive physical therapy evaluations, including OASIS assessments when required
• Design and implement individualized treatment plans targeting functional mobility, pain management, safety, and patient independence
• Deliver skilled interventionstherapeutic exercise, gait and transfer training, neuromuscular re-education, and fall prevention
• Monitor patient progress, adjust plans as necessary, and maintain effective communication with physicians and interdisciplinary team members
• Educate patients and caregivers on home exercise programs, assistive device use, and safety strategies
• Uphold meticulous documentation standards using Home Care Home Base (HCHB)
• Participate in interdisciplinary meetings and case conferences
• Ensure compliance with agency policies, Medicare regulations, and industry best practices

Qualifications:

• Active, unencumbered Physical Therapist licensure in the applicable state
• Minimum one year of recent home health experience as a PT (field-based experience preferred)
• OASIS assessment proficiency required; experience with Home Care Home Base (HCHB) preferred
• Excellent functional assessment and clinical decision-making abilities
• Reliable transportation, valid drivers license, and appropriate insurance
• Able to work independently and manage a flexible field schedule
